Requires Prescription.
In order to process this prescription drug, we need to have a written prescription from your veterinarian. The prescription must include the name of the product(s), the amount and the number of refills that will be allowed along with veterinarian's license, address and phone number.
Please fax the prescription to 402.352.8825. No prescription orders will be shipped until our pharmacist has the prescription on file.
Prescriptions are non-refundable and are valid until they are filled or up to one year from the date written.
We can only ship prescription drugs to the following states:
Alabama, Arizona, Colorado, Florida, Idaho, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maryland, Massachusetts, Michigan, Minnesota, Missouri, Nebraska, New Hampshire, New Mexico, New York, Ohio, Oklahoma, Oregon, Pennsylvania, South Carolina, South Dakota, Texas, Vermont, Virginia, Washington and Wisconsin. |
